If you are a financial aid recipient dropping courses, it is required that you check with Student Financial Services PRIOR to submitting this form to the Office of the Registrar for processing.
The federal government may require you to return some or all of any aid you have received.
CBS policy requires all drop requests to be submitted before 70% of the class has been completed. A total withdrawal can occur in any classes on your schedule that have not exceeded the drop/withdrawal deadline. The form must be received by the Office of the Registrar no later than the close of business on the withdrawal deadline. The effective date of the withdrawal is the date this form is received by the Office of the Registrar. Discontinuance of class attendance or notice to the instructor does not constitute authorized withdrawal. Withdrawals are recorded on your transcript with a grade of "W."
Withdrawing from a course may result in a reduction of your financial aid and a subsequent balance on your account. If you are a veteran or a student receiving financial aid, please consult with the Office of Financial Services before completing this form.
A course withdrawal(s) may affect your student status and eligibility including, but not limited to, the following areas:
You must return any books, equipment, or materials on loan from the College.
It is your responsibility to understand the impact this withdrawal may have on the above for both the current and future semesters. You are strongly encouraged to discuss the potential effects with the appropriate individuals at the College. Review your information (phone, address, etc.) in myPortal and update it as appropriate.

I am aware of all the Academic and/or Financial implications that may arise from the actions requested. I am knowledgeable of the CBS tuition refund policy and understand that fees indicated in the Academic Catalog as non-refundable will not be included in the CBS refund calculation. Refunds of tuition and fees, if applicable, will be based on the date this petition is received in the Office of the Registrar and applicable to the schedule outlined in the Catalog. The refund will be mailed to the address indicated on the electronic student record.
